Description Notification of disqualification from managing corporation - bankruptcy or personal insolvency agreement
Purpose When a person is declared a 'bankrupt', ASIC often receives notification through the lodgement of a copy of the bankruptcy notice issued by the Australian Financial Security Authority (AFSA). Notification of bankruptcy/personal insolvency agreement may also be received by way of this form or letter from the person concerned advising they have been declared a bankrupt or entered into a personal insolvency agreement.

Auditors who are disqualified from managing corporations under Part 2D.6 must tell us by lodging a Form 906

Liquidators must tell us by submitting an RL30.
