Form 145 is used by companies undergoing demutualisation to register a disclosure statement with ASIC.

How to lodge this form

An application for registration of a Demutualisation disclosure statement pursuant to Cl 32 Part 5 Schedule 4 (Demutualisation) should be submitted through the ASIC Regulatory Portal using the ‘Apply for an ASIC approval, consent, statement, nomination, or other’ transaction. Fees will apply to an application and details about payment options are provided in the portal.
