Purpose

This form is for:

  • a company to notify ASIC of the failure to appoint a replacement auditor
  • a member of a proprietary company with more than one crowd source funding shareholder to apply to ASIC to appoint an auditor
  • a member of a public company to apply to ASIC to appoint an auditor.

How to lodge this form

Lodge online

You can complete and lodge the form online in the portal. You may have to pay a late fee if we do not receive your form within the lodging period.

Late fees

A late fee applies to companies that do not notify ASIC of a failure to appoint a replacement auditor within 7 days. See the late fees under 'Late lodgement' (Item 14) in Schedule 2: Registry fees.

Lodging period

A company has 7 days to notify ASIC of a failure to appoint a replacement auditor.
